Hair Transplant

Hair transplant is a surgical procedure that involves removing hair follicles from one area of the scalp (called the “donor site”) and transplanting them to another area where hair has thinned or is balding (called the “recipient site”).

There are two main types of hair transplant procedures: follicular unit transplantation (FUT) and follicular unit extraction (FUE). In FUT, a strip of scalp is removed from the donor site and hair follicles are extracted from this strip. In FUE, individual hair follicles are extracted directly from the donor site using a small punch tool..

Both FUT and FUE are performed under local anesthesia and are generally considered safe and effective procedures for treating hair loss. However, like any surgical procedure, there are risks and potential side effects, such as bleeding, infection, scarring, and unnatural-looking results.

Before considering a hair transplant, it is important to consult with a qualified and experienced hair transplant surgeon, who can assess your candidacy for the procedure and discuss the potential risks and benefits. It is also important to have realistic expectations about the results of the procedure, as it may not be possible to achieve a full head of hair.

FUE HAIR TRANSPLANT

Follicular Unit Extraction (FUE) is a hair transplant technique that involves extracting individual hair follicles from the donor site (usually the back or sides of the scalp) and transplanting them to the recipient site (where hair has thinned or is balding). FUE is one of the most common hair transplant techniques, and is generally considered a safe and effective option for treating hair loss.

During an FUE hair transplant, the surgeon uses a small punch tool (usually less than 1mm in diameter) to extract individual hair follicles from the donor site. The extracted follicles are then transplanted to the recipient site using a needle or microblade. 

One of the main advantages of FUE over other hair transplant techniques (such as Follicular Unit Transplantation, or FUT) is that it does not leave a linear scar on the scalp. Instead, FUE results in tiny circular scars that are less noticeable and easier to conceal. FUE also allows for a faster recovery time, as there is less trauma to the scalp.

However, FUE can be a more time-consuming and labor-intensive procedure than other hair transplant techniques, as each follicle must be individually extracted and transplanted. It is also generally more expensive than other hair transplant techniques.

DHI HAIR TRANSPLANT

Direct Hair Implantation (DHI) is a hair transplant technique that is similar to Follicular Unit Extraction (FUE). In DHI, individual hair follicles are extracted from the donor site using a specialized tool called a Choi Implanter, which allows the surgeon to simultaneously extract and implant the follicles in the recipient site. 

The main advantage of DHI over other hair transplant techniques is that it allows for a more precise and controlled placement of the hair follicles, resulting in a more natural-looking hairline and a higher density of transplanted hair. Additionally, because the Choi Implanter allows for a quicker and more efficient implantation process, the procedure can often be completed in a shorter amount of time than other techniques.

However, DHI can also be a more technically challenging procedure that requires a high level of skill and experience from the surgeon. There is also a higher risk of damaging the hair follicles during the implantation process, which can result in lower graft survival rates.

As with any hair transplant procedure, it is important to consult with a qualified and experienced hair transplant surgeon to determine whether DHI is the best option for your individual needs and goals. SAPPHIRE FUE TRANSPLANT

Sapphire FUE is a variation of the Follicular Unit Extraction (FUE) hair transplant technique that uses sapphire blades to create the incisions for  implanting the hair follicles. In traditional FUE, the incisions are typically made with steel or metal blades.

The sapphire blades used in Sapphire FUE are made of high-quality medical-grade sapphire, which is a hard and durable material that can create cleaner and more precise incisions with less trauma to the scalp. The blades are also thinner and sharper than traditional metal blades, which can result in smaller incisions and less scarring.

The benefits of Sapphire FUE include a faster healing time, less pain and discomfort during and after the procedure, and a reduced risk of infection and other complications. Additionally, the use of sapphire blades can result in a more natural-looking hairline and a higher density of transplanted hair.

However, it is important to note that the success of a hair transplant procedure is not solely dependent on the type of blades used, but also on the skill and experience of the surgeon performing the procedure. BEARD TRANSPLANT

Beard transplant is a surgical procedure that involves transplanting hair follicles from one area of the body (usually the scalp) to the beard area, in order to increase the density and fullness of facial hair. The procedure is generally performed using the same techniques as a hair transplant, such as Follicular Unit Extraction (FUE) or Follicular Unit Transplantation (FUT).

During a beard transplant, the surgeon extracts hair follicles from the donor site (usually the scalp) and then implants them into the beard area using a small needle or microblade. The implanted hair follicles will continue to grow in their new location, producing thicker and fuller beard hair over time.

Beard transplant is typically performed under local anesthesia and can take several hours to complete, depending on the extent of the procedure. The recovery time can vary, but patients are usually able to return to work and normal activities within a few days.

As with any surgical procedure, there are risks and potential complications associated with beard transplant, such as bleeding, infection, scarring, and unnatural-looking results. HAIR TRANSPLANT FOR WOMEN
Hair transplantation is an effective treatment method that creates a positive impact on individuals experiencing hair loss; affecting every aspect of their lives. Although the majority of hair transplantation patients are still men, a high percentage of women have also started to seek hair transplants to cope with hair loss. Hair transplantation is considered a highly important treatment as the final outcome of this procedure will positively contribute especially to a women’s physical appearance. Apart from the negative impacts on physical appearance, hair loss can also lead to feelings of extreme psychological distress. This situation affects women much more than men. It has also been observed that women have the tendency to experience serious psychological problems such as; feelings of inadequacy in a social environment, spending a lot of time and effort to conceal the thinning locks and bald patches or having trouble accepting their hair loss condition. Hair Loss Causes in Women Female Pattern Hair Loss, also known as Androgenetic Alopecia, is the most common cause of hair loss in women. Other potential causes of hair loss in women are: Having a genetic predisposition to hair loss Autoimmune diseases Change in hormone levels Medication Menopause Pregnancy Excessive stress Crash dieting/rapid weight loss Iron Deficiency We can start our analysis by reviewing the most common medical hair loss condition; Androgenetic Alopecia. Androgenetic Alopecia The follicular changes that cause androgenetic alopecia, are similar between male and female pattern hair loss; Progressive shrinking of terminal (thick and dark) hair follicles (hair miniaturization), terminal to vellus transformation (conversion of the thick and dark terminal hairs into very thin and white hairs called vellus), shortening of the hair growth cycle and eventually, the shedding process. Even though male pattern baldness and female pattern hair loss share some similar characteristics, they are actually quite different in many respects. Hair loss in men generally is characterized by a receding hairline (frontal area), affecting the temples (temporal area) and the crown region (vertex area). Women, on the other hand, generally lose their hair diffusely over the crown of the head. It is very rare for women to experience complete baldness. In male-pattern baldness, hair loss causes the hairline to gradually move backwards, creating a receding hairline; whereas a receding hairline in women is very rare and they mostly tend to retain their hairline. Hair loss in women is mostly characterized by a general thinning occurring all over the scalp or localized; affecting only specific regions of the scalp. As it follows a typical pattern, it is much easier to detect male pattern baldness than female pattern hair loss. In addition, as hair loss in women can be triggered by many conditions, it is much harder to diagnose when compared to men.
HAIR TRANSPLANT WITHOUT SHAVING
One of the common questions asked by patients who are planning to undergo a transplant surgery is whether a hair transplant can be performed without shaving the head. Hair transplant without shaving (Unshaven Hair Transplant) is actually something that people would really prefer when undergoing a hair transplant operation. As individuals, we often tend to see our hair as a reflection of our identity, influencing the way we define ourselves; therefore, it is not at all surprising for a person to be reluctant about shaving his/her head when considering a hair transplant. Some people even would their minds when they were told that their heads had to be shaved prior to a hair transplant procedure. At Dr Serkan Aygın Clinic, we conducted research so as to shed light on this particular issue and test whether it was possible to have a hair transplant without shaving. We shall first mention some of the basic reasons why patients hesitate to shave their heads: Can be a traumatic experience for individuals, especially for women who wear their hair long To conceal the hair transplant, as a shaved head would make their hair transplant surgery obvious Restriction due to the nature of the patient’s job, especially for those who work as representatives. Fortunately, it is no longer necessary for an individual to shave his/her entire head prior to a hair transplant with the DHI method, as patients are now offered to choose among four options, consisting of; ‘fully shaved-both donor and recipient sites’, ‘shaved-only the donor site’, ‘partially shaved’-only an undetectable small area in the back of the head’ and if applicable, a ’complete unshaved’ session. The fully shaved hair transplant procedure is the most commonly used method among all options. It is a technique where both the donor area and the recipient site is shaved. The second option is a hair transplant technique that only requires the donor area to be shaved. As the remaining two options; partially shaved and complete unshaven hair transplant constitute the backbone of this article, we will focus only on the latter two methods. Before we proceed further with our discussion, we would like to clarify and emphasize some important points on our topic. Unfortunately, many individuals have fallen into the trap of some hair transplant clinics, which mislead people into believing that they will receive certain treatments that do not even exist or are still impossible to conduct. A clinic that does not actually perform a hair transplant without shaving may falsely promote that they do and once the patient arrives at the clinic, they will tend to persuade him/her to undergo a standard hair transplant by explaining the complications and difficulty associated with an unshaven hair transplant session. Hair Transplant Techniques That Do Not Require A Clean Shave 1. DHI is a technique where the head is ‘partially’ shaved, however, concealed. The term ‘unshaved’ is used to indicate that a small area in the donor site will be shaved but the fact that it is shaven will remain ‘undetectable’. 2. A ‘completely unshaved’ hair transplant, which is a procedure where shaving the donor area for the extraction of follicles is totally eliminated, keeping the hair intact. That’s why it’s called Hair Transplant without shaving. It is important for the patient to be aware of the limitations associated with both of the ‘unshaved’ sessions and to seek consultation from a qualified surgeon to determine if he/she is a good candidate to undergo such a procedure. For example, if the patient has experienced severe hair loss, particularly in the vertex area and requires an excessive amount of grafts (around 3.000-4.000 grafts), his/her head will be completely shaved. The decision is generally made during the planning stage of a hair transplant procedure after the patient has been examined by the physician. 1-Partially Shaved Hair Transplant Patients with longer hair that require approximately 2800 grafts or less for their hair transplant could be a good candidate to have a ‘partially shaved session. It is also referred to as linear shaving, which is performed at the back of the patients head and is either in small lines or in a square shape, positioned in such a way that it enables the hair above to fall over and easily conceal the shaved spot. As it is important to provide sufficient coverage, the doctor may recommend the patient to grow his/her hair slightly longer at a length of 6-7 cm prior to the hair transplant procedure. Advantages of a Partially Shaved Hair Transplant Patients can maintain their original hair in the same style as well as length, during and after the procedure. Allows patients to return back to work shortly after their treatment without revealing they have undergone the procedure. As the number of grafts that will transplanted is fewer, overall recovery time can be shorter. Post-operative scabs and crusts formed in the donor area will not be visible. Disadvantages of a Partially Shaved Hair Transplant This procedure can only be performed on patients who wear their hair long. Fewer number of follicular units can be harvested during any one session. The treatment may require multiple sessions in order to achieve a desirable outcome, however there must be an interval of at least 6 months between 2 sessions. There is the risk of over harvesting as the donor area is limited to a small section. Overall procedure cost is higher. 2- Complete Unshaved Hair Transplant In cases where the patient’s hair is weak and there is the possibility to perform procedures between hair-strands, especially if hair loss is limited to the anterior region or only a small area needs to be harvested, typically up to 1000 follicular units being sufficient for the coverage of hair loss, then the doctor may consider performing a complete ‘unshaved’ session. In a complete ‘unshaved’ hair transplant procedure during the extraction process, a round-tipped rotary punch-generally with diameters ranging between 0.7-0.95mm- is utilized to score around the hair follicle and is rotated a full repetitive 360-degree cycle during penetration of the skin in order to separate it from all tissue attachments. Once the individual follicular units are extracted with the hair remaining at its normal length, they are then cut down to the root. After the strands of hair have been cut off, the implantation process begins, using a choi Implanter pen (DHI Technique), with the grafts being transplanted within the existing hair one at a time into the thinning or balding areas of the scalp. As with any other hair transplant procedure, the patient will still be experiencing shock loss-temporary shedding of the newly transplanted hairs-within the first few weeks after a complete ‘unshaved’ session. Advantages of a Complete Unshaved Hair Transplant Shaving is completely avoided. The patient is required to have long hair similar to the partially shaved hair transplant procedure, however the length can be shorter than 6-7 cm. Post-operative scabs and crusts formed in the donor area will hardly be visible. As the number of grafts that will transplanted is fewer, overall recovery time can be shorter. Disadvantages of a Complete Unshaved Hair Transplant It is a labour intensive and meticulous procedure that requires excessive training, experience and skills of the operating surgeon. In the early version of the DHI hair transplant method, the amount of grafts that could be extracted in one session was lesser compared to the FUE method, but today, owing to the improvements made in the technique, it is possible to extract higher amounts of grafts, providing patients with the necessary hair density and coverage to achieve satisfactory hair transplant results. Even though the patient gets a pre-look right after the procedure, the transplanted grafts will fall out between the 2’nd and 4’th week following the surgery. DHI hair transplant procedure generally takes longer to complete compared to other hair transplant methods. DHI hair transplantation is slightly more expensive than other hair transplant procedures. Preference Ratio of Hair Transplants Without Shaving At Dr Serkan Aygın Clinic, since 1996, we have been performing exclusively on hair transplant procedures. As one of the earliest adopters, we have kept track and witnessed the evolution of hair transplant techniques and with the vast experience we have gained throughout this transformation, performing thousands of surgeries on patients, we have accumulated statistical data on hair transplant operations, which can be seen below: Women and men with long hair constitute the vast majority of patients for the partially-shaved as well as the complete unshaved hair transplant procedures. 94 percent of the patients undergoing a hair transplant favor to shave their head prior to the procedure. 4 to 5 percent of the patients undergoing a hair transplant favor to partially shave their head. And, lastly, if we look at the those who favor to have a complete hair transplant without shaving, we can say that they constitute less than 1 percent of all patients. Some patients who have undergone a hair transplant without shaving their head, express that they were satisfied with the results, however admit that it might have been better if they had shaved their hair down. Unless a complete unshaved session is the only option to select, surgeons who are skilled and experienced in conducting procedures without shaving, generally recommend their patients to shave their hair prior to the operation. As the number of grafts that can be transplanted with a hair transplant conducted without shaving is limited, which will eventually have an impact on the natural appearance, shaving is considered to be highly significant for a hair transplant operation.
